The church’s construction began in 1347 after a reported apparition of the Virgin Mary on June 23 of that year. In 1486, Lorenzo de’ Medici commissioned a new church, and Giuliano da Maiano, a famous architect, designed it. Over time, the church grew in importance. After Savonarola's visit in 1495, the project expanded to include a convent and sanctuary. The expansion accommodated the increasing number of pilgrims visiting the site.
In 1942, the church earned the status of a minor basilica. As visitors enter the sanctuary, they are immediately drawn to the tempietto, a free-standing chapel that holds a stunning fresco of the Madonna and Child by Bicci di Lorenzo. This central feature enhances the space's spiritual beauty.
An altarpiece by Giovanni della Robbia, depicting Christ and John the Baptist in polychrome terracotta, also adorns the sanctuary. Additionally, another Madonna and Child by Fra Paolo da Pistoia adds further religious and artistic significance to the church.
